Discover what does this mean for local people needing help?

Collaboration, community, cooperation – that’s what we would all like to think are the watchwords when we need health and social care at home. But can we be sure that, when contacting social services, we won’t keep getting a recorded message? Or that, when urgent overnight care is required for someone who is terminally ill, we can be confident that such help is available?

Come along on 22 October to a meeting organised by the Putney Society and hear what the people who organise provision of these services can tell us about how this works in practice, and how coordinated and joined-up the services really are.

Speakers confirmed:
● Sandy Keen, Head of Older People’s Services, Clinical Commissioning Group
● Sydney Hill, Head of Early Help and Enablement, Richmond and Wandsworth Councils.

There will also be a speaker from Central London Community Healthcare NHS Trust, and a representative from Healthwatch Wandsworth.

Presentations will cover
● Health and social care at home – including emergency and overnight care
● How different parts of the system communicate to ensure a joined-up service
● How we gain access to such services
● Hospital discharge and rehabilitation and end of life care
● Proposed ‘zoning’ of home care services in Wandsworth

Recent research shows that more and more older people will require help in the future. Let’s be assured that the services needed will be there for us all.
